We Offer 5 Core Services (Free of Charge):

  • Information & Referral - link individuals with appropriate agencies and services. 
  • Peer Support - individual or group meetings.
  • Transition - assisting individuals with significant disabilities from restrictive settings such as a nursing home or with youth who are eligible for an IEP or 504 plan (post-secondary life). 
  • Advocacy - educate consumers and community regarding roles and responsibilities under the Americans with Disabilities (ADA) Act. 
  • Independent Living - assist individuals in developing skills such as personal care, financial management, social skills, and household management.

"Need-A-Ride" Transportation (Brevard Only): free transportation is provided for registered consumers.

Durable Medical Equipment: loaned to consumers in need.

Items such as:

  • Wheelchairs
  • Walkers, Canes
  • Bedside Commodes
  • Shower Chair, Portable Ramp

by the U.S. Department of Health and Human Services (HHS)





Additional Info

Assisting people with disabilities to live independently through Transitions & Diversions, Advocacy, Information and Referrals, Independent Living skills training, and Peer Support. Additional services available are: loans of durable medical equipment - assistive devices such as walkers,  transportation services (Brevard Only).
